When you want The Yomiuri Shimbun, Tokyo, to disclose the personal information it collects about you, or take other steps concerning such information, please follow the following procedures:

(1) Download
Download an application form you want to apply and print it out. You cannot enter any information on the screen-based file.
(2) Or send postal mail
Send to the following address a postal mail providing your name, address and phone number, enclosing an ¥80 stamp for reply and specifying the type of personal data (examples: "the list of participants in Event ABC held in January 2005" or "the list of applicants in the 2004 XYZ Essay Contest") you want us to disclose.

- Return the application form
Fill out the necessary application form and send it to the abovementioned address by postal mail. In the case of "DISCLOSURE" or "NOTIFICATION OF PURPOSE FOR USING PERSONAL INFORMATION," deposit ¥500 in the designated postal money order account at Japan Post. In the case of "CORRECTION" or "SUSPENSION OF USE," pay ¥430 into the designated PMO account. The deposited money will be used as a postal fee of ¥430 yen for sending you a reply by Kan-i registered mail as well as a related fee of ¥70 involved in the DISCLOSURE and NOTIFICATION categories. Depending on circumstances, you may be requested to submit some document identifying yourself.
* The personal information entered in the said application form will not be used for purposes other than processing your request.
